How this website handles appointment enquiries.

Information you provide

The appointment form asks for your name, email address, optional phone number, preferred service and appointment format, and an optional message. These details are emailed to createpsych@gmail.com so our admin team can respond and help arrange an appointment. The form does not create a client account or store your enquiry in a website database.

Keeping your enquiry brief

Please do not send bank account or credit card details, Medicare numbers, copies of identity documents, detailed medical histories, referral attachments or other sensitive personal information through the website enquiry form. Keep your message brief and include only what admin needs to respond to your enquiry.

Our admin team can explain what information or documents are needed for your appointment, provide any forms you need to complete, and guide you on how to submit them.

Website services

The site is hosted through Bluehost. Enquiry email is handled through the practice’s Gmail inbox. These service providers may process technical information or enquiry data as part of providing their services. The website also loads fonts from Google Fonts and links to Facebook. Visiting an external service is subject to that service’s privacy practices.
